The following clarifications and revisions for the above-referenced project are hereby incorporated by
Addendum #1 to the previously issued bid documents.

Project Manual – Section 010 – Advertisement for Bids - Revision: The acceptance of bids will be
officially closed at 2:00pm on December 18, 2024. The attached Addendum #1 Section 010
supersedes the previously issued Advertisement for Bids in its entirety.
Project Manual – Section 020 – Scope of Work - Revision: The Bid Closed/Opened & Read Time will
be at 2:00pm on December 18, 2024. The attached Addendum #1 Section 020 supersedes the
previously issued Scope of Work in its entirety.
Project Manual – Section 210 – First-Tier Subcontractor Disclosure Form - Revision: The Bid Closing
Time is at 2:00pm
on December 18, 2024. The attached Addendum #1 Section 210 supersedes the
previously issued First-Tier Subcontractor Disclosure Form in its entirety.
Project Manual – Section 230 – Schedule of Bid Items - Revision: The type of unit for Bid item #7
Foundation Investigation has been updated per attached Addendum #1 Section 230 - Schedule of Bid
Items.

#A1 and #B1 have been updated per attached Addendum #1 Section 230 - Schedule of Bid Items.

Contractor Question – Section 00221.05 (a) makes reference to the Owner and Contractor agreeing on
staging methods and access provisions for businesses before the Contract is signed. Can you provide
clarification? Can you provide minimum requirements for this so that we know what the Owner wants
before they sign the contract? Clarification: As noted in Section 00221.05(a), the Owner and
Contractor must agree on staging methods and access provisions for businesses before the contract
is signed. At a minimum, the agreed-upon provisions should ensure continuous access to businesses,
even during construction. This may involve temporary access routes, alternate entry points, or
scheduling specific work outside of business hours. The plan should minimize disruption to
businesses while maintaining efficiency. Please communicate with the Owner and the City during pre-
construction planning to finalize specific details and address any unique site conditions.

Contractor Question – Section 00221.9 (c) are there minimum requirements for the Pre-Construction
Video equipment or the video itself? Clarification: Per Section 00221.9(c), there are no explicitly
defined minimum requirements for the pre-construction video equipment or the video itself. However,
the video must provide clear and comprehensive documentation of site conditions to ensure accurate
records. It is recommended to use high-resolution video equipment with stability features (e.g., tripod
or gimbal) to capture detailed footage. Videos should be well-lit and include date, time, and location
metadata for reference.

Contractor Question – Section 00310.9 (q) Do you have a quantity of existing buildings that we will
be doing this investigative work or should we use the 53 addresses given to us in the bid items?
Clarification: Please note that the type of unit for Bid Item 7 has been revised to "each" and has been
reissued with the updated Schedule of Bid Items for your reference.

Contractor Question – Section 00330.41 (a) (15) requires the Contractor to repair damaged subgrade
as directed by the Engineer without additional compensation. Can you clarify when the repairs are
the responsibility of the Contractor and when will the Subgrade Stabilization item be used?
Clarification: The responsibility for repairing damaged subgrade as outlined in Section
00330.41(a)(15) falls on the Contractor when the damage is caused by the Contractor’s operations,
such as failure to follow specified construction methods or inadequate protection of the subgrade.
These repairs are considered part of the contract scope and will not result in additional compensation.
The Subgrade Stabilization item will be used when stabilization is required due to unforeseen site
conditions, such as naturally weak soils or excessive groundwater, that were not identifiable during
the planning phase. This item is specifically intended for issues outside the Contractor’s control and
will be compensated accordingly.

Contractor Question – The bid item description for the Asphalt lists Level 2 but the typical sections
show a Level 3. Can you clarify? Clarification: The typical sections shown on the plans are accurate,
and the Schedule of Bid Items #A1 and #B1 have been updated accordingly. The revised Schedule of
Bid Items will be reissued with Addendum #1.

Contractor Question – Section 00756 Plain Concrete Pavement: Which smoothness testing method will
be required for this project? Clarification: For this project, smoothness testing for Plain Concrete
Pavement will involve both straightedge testing and International Roughness Index (IRI) profiling per
Section 00756.55(c). Areas that are excluded from IRI requirements, such as ramps, low-speed auxiliary
lanes, and areas within 50 feet of bridge approach slabs, will still need to meet the straightedge
smoothness criteria outlined in Section 00756.55(a).

Contractor Question – Will there be a bid item for sidewalk removal or is this work included in the
General Excavation item? Clarification: Sidewalk removal is not explicitly listed as a separate bid item
in the Schedule of Bid Items. Per Section 00310.44, excavation required for the removal of structures
and obstructions, including sidewalks, is considered incidental to the removal work unless otherwise
specified. Therefore, this work will be included in the General Excavation item unless stated otherwise
in an addendum or special provision.

Contractor Question – Can you confirm that Item #45, Variable Height Curb, is paid by the square
foot? Clarification: Yes, Item #45, "Variable Height Curb," is paid by the square foot, as specified in
the Schedule of Bid Items. The listed quantity is measured in square feet, and payment will be based
on this unit. Please refer to the bid schedule for further confirmation.

Contractor Question - Is there a pay item for replacing weep hole piping? Clarification: The
Schedule of Bid Items #26 and #27 includes provisions for weep hole piping (roof drain).

Contractor Question - Does detail 4/B on sheet C5.5 occur on this project? Clarification: Yes, this
detail is applicable and is called out for use on the south side of the bridge on Main Street. Please
refer to sheet C4.0 for additional information.

Contractor Question - Sheet C4.0, is the shaded area for Construction Note #402 paid for under one
of the asphalt bid items in the Alternates? Clarification: Yes, the shaded area noted in Construction
Note #402 will be paid for under the Asphalt bid item in the Alternates.

Contractor Question - Where will Bid Item #4 be required? Clarification: Bid Item #4 will be required
at all locations where water and storm utilities are being constructed outside of the new pavement
footprint. The Contractor is responsible for determining the means and methods for utilizing
temporary trench resurfacing as needed.

Contractor Question - Are there any permits or fees the City will be responsible for? Clarification:
Yes, the City will obtain and pay for the ROW permit and the 1200-C Permit for the Contractor. The
Contractor will be required to adhere to all applicable permit requirements.

Contractor Question - Is 3⁄4”-0 aggregate acceptable to use instead of 1”-0? Clarification: Per ODOT
specifications, either 1”-0 or 3⁄4”-0 aggregate is acceptable for use on this project.

Contractor Question - Note 39 on sheet C0.1 says that the Contractor is to reimburse the City for
costs to flush, test, and disinfect the waterline. Can you let us know what the City charges for this
work? Clarification: Note 39 on Sheet C0.1 specifies that the Contractor is responsible for
reimbursing the City for the costs associated with flushing, testing, and disinfecting the waterline.
While the project plans do not specify these costs, they are determined by the City of Cottage Grove
based on their current rates and procedures. The Contractor should contact the City of Cottage
Grove Public Works Department directly to confirm the exact charges for these services.

Contractor Question - I was wondering if you had an engineer’s estimate for the E. Main Street
Revitalization Project #22-001H? Clarification: No, an engineer’s estimate is not available for this
project.

NOTICE IS HEREBY GIVEN THAT Sealed bids for the E. Main Street Revitalization Project, City of Cottage Grove, Oregon, shall be addressed to the City Engineer, Damien Gilbert, PE, 400 E. Main Street, Cottage Grove, OR, 97424 and will be received at this location. Acceptance of bids will be officially closed at 2:00 pm Pacific Time, December 18, 2024, and immediately thereafter the bids will be publicly opened and read at Cottage Grove City Hall, 400 E. Main Street, Cottage Grove, OR, 97424. The bid shall be submitted under sealed cover and marked with the Contractor’s name and project name. Bids received after the date and time fixed for opening will not be considered. No electronic or facsimile submittals will be accepted.

The project involves the construction of E. Main Street that include: waterline and stormwater installation, grading and asphalt paving, concrete sidewalks, concrete pavement, signing and striping in Cottage Grove, Lane County, Oregon. The work involves excavating, grading, stormwater service pipe and water service line installation, asphalt and concrete paving, restoration, electrical work, street trees planting, signing and striping.

The Project Manual including; Plans, Specifications, Agreement, and Bid Forms are available at the City of Cottage Grove’s website https://www.cottagegroveor.gov/ where they can be viewed and printed. Any Addenda will also be posted on the city’s website City of Cottage Grove’s website https://www.cottagegroveor.gov/. All prospective bidders must be added to the Plan Holders List by sending an e-mail with company contact information to jessicam@branchengineering.com with the project title in the subject line. Prospective bidders must be on the Plan Holders List for their bid to be considered. Bidders are responsible for checking the website for addenda and changes prior to submitting bid, however notification of Addenda issuance will be issued via e-mail to the addresses listed on the Plan Holders List. No hard copy of the bid documents will be available. A Mandatory Pre-Bid Meeting will be held at 400 E. Main Street, Cottage Grove, on Wednesday, December 4 th , 2024 at 3 PM.

Bidders must be pre-qualified with the City of Cottage Grove in accordance with Oregon Revised Statutes (ORS) Chapter 279C.430 by submitting a completed Oregon Department of Transportation form on or before 5:00 p.m., Monday December 16 th, 2024. Bidders must submit the Pre-qualification form to Branch Engineering by emailing jessicam@branchengineering.com. Bidders and/or subcontractors must be prequalified in street repair construction, wastewater, and water construction, earthwork and drainage to complete the project. You may pre-qualify with the Oregon Department of Transportation at 121 Transportation Building, Salem, Oregon 97310.
