RE: SOUTH SIXTH STREET IMPROVEMENTS - 2017 PROJECT

 

Dear Dennis,

 

Your company was the successful bidder on our SOUTH SIXTH STREET IMPROVEMENTS - 2017 project, and the Cottage Grove City Council awarded the contract in the amount of $ 427,365.36 plus a flagger cost of $62.00 per hour to you at their regular meeting of Monday, September 11, 2017.

 

Enclosed are four (4) sets of the contract documents for the above referenced project.  Please execute and return all four (4) sets to us.  Each set should be accompanied with a copy of your Performance Bond, Payment Bond, Public Works Bond, and Insurance Certificate.

 

You will also find enclosed an original of the following forms:

 

WH-81 – Complete Section 3, Prime Contractor Information, and submit to the City of Cottage Grove.

 

WH-38 – Complete this form, and as the prime contractor you are to provide copies of this form to every subcontractor on the project.  At your option you may use your own form to submit this payroll information.

 

Contractors are no longer required to submit copies of the Payroll/Certified Statement Form WH-38 to the Bureau of Labor and Industries.  The statements must be submitted to the City.

 

Upon receipt of the following:

 

Signed contract documents including:

Performance Bond
Payment Bond
Public Works Bond
Insurance Certificate

 

 

 

Copy of Form WH-81

 

The City will execute the contract documents and prepare a “Notice to Proceed” with the work in compliance with the contract workdays.  We will also schedule a pre-construction meeting to discuss the project with you and other involved parties.
